Do you take insurance

As of January 20, 2022, Kristi is in-network with Humana commercial, United Healthcare/Optum commercial and Aetna commercial insurance. If you wish to use your health insurance benefits, you will need to contact your insurance company to determine your coverage and any deductibles or co-pays, which will be requested at the time of service. You will be responsible for any denial of insurance coverage.

What if I want to use out-of-network insurance?

For out-of-network insurance, we request payment up front. After each session, the office will provide you with a Superbill to request reimbursement from your insurance provider. Reimbursement of out-of-network coverage varies; please contact your insurance company to determine OON benefits. 

What are your fees? Can I use a credit card?

Initial appointment (90791): $150.00

45-50 minute session (90834): $110

55-60 minute session (90837): $130

80 minute session (+99354): $150

I accept cash, check, HSA, FSA or credit cards.

Do you offer sliding scale?

Kristi is happy to discuss an income-based sliding scale fee if you are do not have or are unable to use insurance.

How many sessions will I need?

At or before (via phone consultation) your initial session Kristi will discuss proposed length of treatment and will provide a Good Faith Estimate of cost of treatment. Please note that your goals may change throughout treatment. You may request an evaluation of progress at any time.
